E-Rate Survey

A survey was sent home with students today. The date on the survey is incorrect. Originally, we were supposed to receive/ complete the survey in February. With all the inclement weather that occurred, we did not get it out to you. We need your help. Please take just a few minutes to answer the questions. We would greatly appreciate it.

The following is the letter from Mr. Raines, our Superintendent, explaining the survey.


Dear Parents,

Our schools participate in a federal government program which provides our schools with substantial discounts on our telephone and internet bills and on computer equipment purchases.

These discounts reduce the costs of our telephone and internet services along with the equipment we use to build and maintain the computer networks that link our classrooms and allow students access to the internet.  The amount of the discount is based on the average income level of all our student’s families.  These discounts save the district and taxpayers a substantial amount of money and allow us to increase the level of technology we offer your children. 

We need your help in qualifying for the largest discount allowable by providing some general information.  Please take a minute to complete and return the attached survey to your student’s classroom teacher.  This information will remain confidential and will be reported only as a total group, not by individual families.  It will only be used to increase our technology discounts and for no other purpose. 

IT IS IMPORTANT TO COMPLETE AND RETURN THIS FORM EVEN IF YOU ANSWER NO TO ALL THE QUESTIONS.

Thank you for helping our schools stretch their dollars in these difficult budget times to best serve all our students.  Please return this survey to your child’s homeroom teacher as soon as possible.  If you have any questions, please call your school principal’s office or your child’s teacher.
